TNTFI Enrollment Process for Employers **This page is for CDDP/Brokerage personnel use only**
We are pleased to announce that the New Employer of Record (EOR) Request Form is now available online. When you click the EOR Enrollment Request link below it will redirect you to a secure website link to complete the request form.* This online form will allow the CDDP/Brokerage to complete and submit their requests electronically, directly to TNT. If this is your first time using the online EOR Enrollment Request form, please read the TIPS and FAQ's sections below completely.
ALL ENTRIES MUST BE IN UPPER CASE. PLEASE TURN CAPS LOCK ON.
No special characters (including periods, commas, etc.) are allowed in the name or address fields. (# and - are allowed)
Required fields that do not pass validation formatting will be outline in burnt orange. An example would be that the PRIME# must be exactly 8 characters long, while a CPMS number must be exactly 6 numeric characters.
Hover over the outlined field with your mouse/pointer to view a specific error message.
Once all required fields have been completed, click the "Create Enrollment Request" button. This will notify TNT of the request and will send an email to the requestor notifying them we have received the request.
Q: I have completed the online form but the “Create Enrollment Request” button is shaded and will not let me submit? A: The “Create Enrollment Request” button is only activated to allow submission when all required fields have been completed. If the “Create Enrollment Request” button is not activated please review the form again. Hover over each field outlined in burnt orange with your cursor for additional information on required correction before submitting. Once the needed corrections have been made, the button will activate.
Q: Can I still submit my request using the fillable PDF form on SharePoint, or by fax? A: Yes, the online Employer Enrollment request has been provided for your convenience. If you are unable to use or access the form for any reason you may continue to submit your requests via SharePoint or Fax.
Q: How will I receive my prepopulated Employer Enrollment Forms, once the request is submitted? A: Once a complete and correct request is submitted, your prepopulated Employer Enrollment Forms will be uploaded to SharePoint within two business days. An email notification will be sent to the requestor to inform you of its location. In the event clarification or correction of information submitted is required, an email will be sent to the requestor for the required information.
Q: If the Customer/Employer is a transfer from another CDDP/Brokerage do I need to complete a request form? A: Yes, under the “Request Type” drop down select “Transfer from another CDDP or Brokerage”. Then select the Brokerage or CDDP that the Customer/Employer is transferring FROM. We will research these requests and send notification. If a new set of enrollment forms is required, we will prepopulate the enrollment forms and upload them to SharePoint sending notification of its location. If a new set of enrollment forms is not required, we will send notification informing you of this.
Q: How long do I wait to follow-up? A: Once a request has been submitted you should receive notification within two business days. If you have not received any notification by the end of the second business day please feel free to contact TNT at OregonCP@tntmanagement.com or (503)463-0134 to follow up on your request.
Q: How do I make corrections after hitting submit? A: It is important that you review all information for accuracy prior to submitting the request. If you have submitted your request with an Error please contact TNT immediately via email at OregonCP@tntmanagement.com
TNT has provided the online EOR enrollment request form as a convenience for the CDDP’s and Brokerages. With so many different entities using a wide range of systems and settings, it may not always be possible to access the form. If this is the case you may continue to submit the Employer Enrollment Request as you have in the past utilizing the fillable PDF version and returning it TNT via SharePoint or Fax using the fax number listed on the bottom of the form.
